What we do:
Community Support Facilitators (CSFs)
CSFs will identify and encourage individuals on an individual basis to access community-based services and activities that meet their needs in a person-centred approach that will be time limited. Individuals will then be reviewed and signposted to the most appropriate support within Lewisham.
Community Development Workers (CDWs)
CDWs will support, improve and create networks that work with local voluntary and charity sector organisations to develop services and cross support activities. CDWs will also develop intelligence on local services that can be shared across the borough that will support the CSFs and community organisations to provide specialised ongoing support structures for the individuals.
Community Connectors (volunteers)
Connectors provide time limited support to vulnerable people by visiting them at home to help linking them to local social activities and support services. Volunteers also benefit from a valued relationship.
